【摘要】:1.1ThisguidecoversthefunctionalelementsanddatarecordsofprehospitalHealthStatusInformationServices(HSIS)neededtoprovideindividualhealthstatusdataforHSISsubscribers.WhenanHSISsubscriberexperiencesamedicalemergencyandbecomesanEMSpatient,aprehospitalEMScareprovidercanrapidlyaccesstheindividual''shealthstatusdatabymeansoftelecommunications.AccesstothisdatawillenabletheEMSprovidertoimprovepatientassessment,andtherebyrendermoreappropriatetreatments.ThiswillimprovetheEMSprovider''sabilitytostabilizetraumaandotheremergencymedicalconditions,andtorestoreandsustainvitalfunctions,whileavoidingtreatmentsthatmayaggravatetheseverityofthemedicalemergencybecauseofpreexistingconditions.1.2Inadditiontoimprovingon-siteassessment,thisguidewillfacilitateimprovedon-linemedicaldirectionofprehospitalEMScareproviders,particularlyforpersonsexperiencinglifethreateningmedicalemergencies.1.3Healthstatusrecordsprovideachronologyofaperson''shealth/medicaldata,includingpastdiagnosisandtreatments.Thedataintheserecordsprovideavitallinkbetweenthepersonexperiencingamedicalemergency,theEMScareprovider,andsubsequentemergencyservices.Inordertoprovidethemostinformedcare,EMScareprovidersandpersonsprovidingEMSmedicaldirectionneedtobeawareoftheinjuredorillperson''shealthstatus.1.4Thisguidedescribestheminimumrequirementsforcompiling,updating,computerizing,andstoringindividual''slongitudinalhealthstatusdatainauthorizedrepositories,soastoprotectpatientprivacyandconfidentiality.ThisguidealsodescribesrequirementsforprovidingauthorizedaccessandrapidtransmittalofthedatatoattendingEMScareprovidersinmedicalemergencies.1.5WhilethisguideaddressesdataneededforprehospitalEMS,thereisalsoarecognizedessential,butlargelyunmetneedforsimilarpatienthealthstatusrecordsforemergencymedicalcareofpatientsinhospitalemergencydepartmentsandindefinitivemedicalcarefacilities.Manydevelopmentprojectsareinprocesstoaddressthisunmetneed.Whenavailable,suchpatientrecordsarereviewedbyattendingphysicians,inadvanceofhospitalemergencymedicalcare,toquicklyaccesspatienthealthstatusdatathatisneededforimprovedpatientassessmentandtreatmentandavoidanceoftreatmentswhichmaybecontraindicatedbypreexistingconditions.1.5.1Futurechangestothisguidewillresultinhealthstatusinformationrecordsforprehospitalemergencymedicalcareandanalogousinformationsystemsforhospitalemergencymedicalcare,harmonizedwitheachotherandwithfuturestandardsforcomputerizedlongitudinalhealthcarepatientrecords(seeGuidesE1744andF1629)whicharebeingdevelopedbyASTMCommitteeE31.1.5.2ThisguidedescribesrequirementsthatarebasedoncurrentASTMmedicalinformaticsstandardsandwillbeupdatedtoharmonizewithfutureversionsoftheserapidlyevolvingstandards.1.6ThescopeofthisguideincludesharmonizationofthedefinitionsofprehospitalemergencymedicalservicesdataelementdefinitionsusedinthisguidewithdefinitionsusedinotherASTMstandards.ThedefinitionofdataelementsinthisguidewillbethesameasthedefinitionofthedataelementinotherASTMstandards.IncaseswhereadataelementusedinthisguidedoesnotappearinanotherASTMstandard,theguidewillusethedefinitionspecifiedforfederalhealthservicesinformationsystems(4,5).
